چکیده
110/60 mmHg and pulse rate 130/min and regular. Results of laboratory investigations were normal. S. flexneri type 2a was later isolated from the stool. Treatment was started with ampicillin 250 mg every 8 hours orally. Six hours after admission she had a grand mal seizure and shortly thereafter became apnoeic and unresponsive to noxious stimulation. The pupils were widely dilated (6 mm) and fixed, and there was no response to caloric or oculovestibular testing. Cerebrospinal fluid analysis was normal. Computerised tomography scan of the brain showed diffuse areas of low density throughout the brain. Five hours later the electroencephalogram was isoelectric. At necropsy the entire bowel was congested and the colonic mucosa was infiltrated by polymorphonuclear cells. Extensive necrosis was present throughout the brain and coning of the cerebellum was noted. The remainder of the organs seemed normal.

In this study the data on 115 cases of congenital adrenal hyperplasia (CAH) who were followed in the Pediatric Endocrine Clinic at Nemazee Hospital, Shiraz will be reported. Among these cases 51 were male and 64 female. The most common type of CAH in these patients was the salt-losing type of 21 -hydroxylase deficiency (85 .2%). ll-hydroxylase deficiency was present in 13.04% of patients. ...
متن کاملCongenital adrenal hyperplasia--a clinical and genetic survey. Are we detecting male salt-losers?
In a genetic and clinical study in Wales of congenital adrenal hyperplasia due to 21-hydroxylase deficiency, 26 cases were found in the period 1966 to 1977. No one was known to have died from the condition in the period. There were 14 female and 5 male salt-losers, and 5 female and 2 male nonsalt-losers. The discrepancy between the sexes suggests that some cases of congenital adrenal hyperplasi...
متن کاملAldosterone Secretion Rate in Congenital Adrenal Hyperplasia. a Discussion of the Theories on the Pathogenesis of the Salt-losing Form of the Syndrome.
The most common form of the congenital adrenal hyperplasia syndrome is caused by an inefficient enzymatic hydroxylation of the adrenal steroids at the 21 position (1-4). The symptoms of this disease can be explained by a deficiency in cortisol secretion resulting in an increased ACTH output and an overproduction of androgens and cortisol precursors by the hyperplastic adrenals (5, 6). About a t...
متن کاملPlasma renin activity in the management of congenital adrenal hyperplasia.
Fourteen children with salt losing and five children with non-salt losing congenital adrenal hyperplasia were studied. Venous samples were collected for measurement of plasma renin activity, serum 17 alpha-hydroxyprogesterone, testosterone, sodium, and creatinine. Overnight urinary sodium and creatinine excretions were measured after collection on an outpatient basis. Eight 'salt losers' had a ...
